Medicare Sustainable Growth Rate

Key Information: 
On March 31, the U.S. Senate passed a 12-month patch to avert a 24.1 percent cut to Medicare physician payment per the Sustainable Growth Rate (SGR) formula. Last week, the U.S. House passed the legislation by a voice vote. The bill will now go to the President and he is expected to sign it into law. Read a summary of the legislation.  

The current patch provided for a 0.5 percent update which expires on March 31, 2014. The proposed patch legislation extends the 0.5 percent update until Dec. 31, 2014, and includes a 0 percent update from Jan. 1, 2014 to April 1, 2014. The legislation also includes other provisions that have typically been included with previous SGR patch legislation, such as an extension of the geographic practice cost index (GPCI) floor.

Additionally, as an inducement for physician organizations to support the proposal, the patch legislation would delay the transition to ICD-10-CM/PCS until Oct. 1, 2015.

While ASA supports efforts to prevent the implementation of the 24 percent payment cut, ASA opposed the current patch because of the inclusion of provisions that could adversely impact Medicare payments to anesthesiologists.  Previous patches have blocked the implementation of the SGR cuts without the inclusion of add-on provisions that would adversely impact payments to physicians.

ASA's Position on the Sustainable Growth Rate (SGR) formula: 
ASA supports full repeal of the SGR with replacement by a mechanism that accurately reflects the costs of providing care. Short of full repeal, ASA supports proposals that 1) provide for appropriate payment updates; 2) do not exacerbate projected future payment cuts and 3) begin to address the magnitude of the projected payment cuts – the “SGR debt.”

ASA opposes the sequestration cuts.
